The Applicant acknowledges and declares that the data shared with Op. Dr. Koray Kırbıyık (www.koraykirbiyik.com) is accurate, that they will update such data in the event of any change, and that our clinic shall not be held responsible for any damages that may arise from the failure to update this data or from the provision of inaccurate data.

The Applicant expressly consents to the collection, recording, processing, storage, use, and transfer/sharing by our clinic of the personal data shared with our clinic for the purposes of carrying out appointment procedures, providing medical services, conducting treatment processes, fulfilling our legal obligations, and serving as a resource for services that may be offered in the future. The third parties to whom the Applicant has consented to the transfer and sharing of their personal data include, but are not limited to, the following:

Domestic and/or foreign third parties with whom our clinic has a service relationship, and research, promotional, and consultancy companies that process data on behalf of our clinic. This information shall be shared with third parties that act in compliance with the relevant legislation on data protection and security, solely for the purpose of fulfilling the rights and obligations of the parties in connection with the medical service relationship established with the Applicant, and only when needed and to the extent necessary.

Rights Under the Personal Data Protection Law (KVKK)

Pursuant to Article 11 of the Personal Data Protection Law, the Applicant may exercise the following rights by submitting a wet-signed application to our clinic's address at Greentown Plaza, Gazi Mustafa Kemalpaşa Mah. Narin Cd. No: 9/1 Kat: 12 Daire: 44 Çerkezköy / Tekirdağ :

• To learn whether their personal data is being processed,
• To request information regarding such processing if their personal data has been processed,
• To learn the purpose of processing their personal data and whether it is used in accordance with that purpose,
• To know the third parties to whom their personal data is transferred, whether domestically or abroad,
• To request the correction of their personal data in the event that it has been processed incompletely or inaccurately,
• To request the deletion or destruction of their personal data within the framework of Article 7 of the KVKK,
• To request that the correction, deletion, or destruction of their personal data be notified to the third parties to whom the personal data has been transferred,
• To object to the occurrence of a result against the person themselves arising from the analysis of the processed data exclusively through automated systems,
• To request compensation for the damage in the event that they suffer harm due to the unlawful processing of their personal data.
